Case Presentation

  • A 90-year-old female presented with progressively worsening dyspnea on exertion and chest pain over the last week.
  • Medical history: Severe AS s/p TAVR 3/8/16 (EVOLUT-R 29mm), Hypertension, Hyperlipidemia, Peripheral arterial disease with claudication, and chronic atrial fibrillation on Xarelto.

Coronary angiogram was done by using Ikari right catheter 1.5 via radial approach. Angiogram of left and right system showed no significant coronary artery disease.

Learning Points

  • If we used radial approach in post TAVR case, Ikari right guide catheter 1.0 or 1.5 should be the first catheter choice.
  • JR4 can also be used for RCA angiogram.
