  • If commissure can end up positioned directly in front of the coronary ostium, coronary can be engaged from diamonds on either side of the ostium, as co-axial engagement would not be possible.
  • Due to its design and reach, an Ikari Right guide is preferred for selective coronary engagement in these cases (Tool-box).
  • In balloon-expandable valve, a commissural post can end up positioned directly in front of the coronary ostium near or at the level of the sinotubular junction.
  • In this scenario, a coronary can be engaged from the open cell on either side of the ostium, as co-axial engagement would not be possible.
